Helping us better understand and support our community

East Wellington Family Health Team (EWFHT) is committed to understanding and supporting the unique needs of the patients and communities we serve.

We know that factors such as language, income, education and life experiences can affect health and access to care. To help us better understand the population we serve, EWFHT is piloting a short Health Equity and Sociodemographic Survey with patients from a selected physician's patient panel.

The survey takes approximately 2–3 minutes to complete.

This work supports EWFHT's ongoing efforts to advance health equity and is aligned with Ontario Health's approach to understanding social determinants of health and using sociodemographic information to identify disparities and inform health system planning.

What will the survey ask?

Some of these questions may feel personal. You are always in control of what you choose to share.

Your participation is voluntary

Participation in the survey is completely voluntary.

You may choose “Prefer not to answer” for any question, and choosing not to participate will not affect the care you receive from EWFHT.

Your responses are anonymous

Survey responses are completely anonymous and will not be linked to your medical record.

The information collected through the survey will help us understand our patient population and inform future planning and improvement efforts.

About the Pilot

This survey is being introduced as a pilot with a selected physician's patient panel. This means not all EWFHT patients will receive a survey invitation.

Starting with a small pilot will allow us to learn from the experience, gather feedback and identify opportunities for improvement before determining next steps.
